MEMBERS: You must complete an Authorization to Release Health Information form if you want to grant permission for us to release private information about your care. Unless you file this form, we can’t discuss your health—even with your trusted family and friends.

As of Jan. 1, 2010, we now have a new authorization form that enables us to release details on the following conditions: alcohol and/or substance abuse; mental health; HIV/AIDS; genetic diseases or tests; family planning (including abortion); and sexually transmitted diseases. Any forms you may have submitted in the past do not permit us to release this protected information. You may download a copy of our authorization form at our Web site or call member services to get a copy.
