No Special Paperwork for Referrals To help assure that the medical services you receive are appropriate and coordinated, CDPHP requires members with HMO coverage to have a primary care physician (PCP). Your PCP will direct your care and refer you to specialists within the CDPHP network as needed.
When you are referred to a specialist, CDPHP does not need a form or referral number. Your PCP can direct you to another participating physician without filing any special paperwork. All we ask is that when you go for your specialist visit, please tell the receptionist the name of the doctor who referred you. Please note, inpatient hospitalization and certain other services continue to need preauthorization by CDPHP. Your doctor will work with CDPHP to obtain this clearance before your hospital stay.
