A CDPHP inpatient care coordinator will work closely with your health care team, reviewing your medical records and plan of care. This way we can be sure that you are receiving the right care, in the right setting, at the right time.

Your inpatient care coordinator can also serve as a resource to ensure a smooth transition to home or another level of care.
